About the Role

We are seeking a highly skilled Care Coordinator to join our team at the State of Georgia. In this role, you will play a critical part in ensuring the highest quality care for our clients.

As a Care Coordinator, you will be responsible for assessing client health problems and needs, developing and implementing nursing care plans, and maintaining accurate medical records.

You will also administer nursing care to ill, injured, convalescent, or disabled clients, utilizing your expertise to provide exceptional patient outcomes.

Key Responsibilities
  • Assess Client Needs: Conduct thorough assessments of clients' health problems and needs, identifying areas for improvement and developing targeted care plans.
  • Nursing Care Planning: Create and implement individualized nursing care plans that address clients' unique needs and promote optimal health outcomes.
  • Medical Record Maintenance: Accurately maintain and update clients' medical records, ensuring seamless communication with healthcare providers and stakeholders.
Requirements
  • Education: Bachelor's degree in Nursing or related field required.
  • Licensure: Active Registered Nurse (RN) license in good standing.
  • Experience: 1+ year of experience as a RN in a healthcare setting preferred.
