Job Summary:

The Support Coordinator role at United Cerebral Palsy of Georgia involves assisting adults with intellectual and developmental disabilities in their day-to-day lives. This position requires a strong understanding of the unique strengths and challenges faced by each individual, tailoring support to promote personal growth and independence.

Duties/Responsibilities:

  • Provides transportation and supervision for individuals as required to meet employment, recreational, and leisure activities.
  • Ensures safe transport of residents, including performing tie-downs and seatbelt usage.
  • Performs housekeeping responsibilities, including kitchens, bathrooms, windows, bedrooms, and floors.
  • Maintains all standards when preparing meals and managing diets, adhering to guidelines for safe food handling and ensuring adequate supplies.
  • Provides personal care support, including changing, dressing, and other ADLs.
  • Completes documentation and notes as required using the Therap system and other programs.
  • Ensures goals are being completed according to each individual's ISP.
  • Conducts evacuation drills and records results.
  • Assists in planning and facilitating person-centered outings.
  • Completes behavioral tracking logs as required.
  • Follows agency, State, and Federal laws and policies, utilizing PolicyStat as needed.
  • Completes required training, using Relias to complete or register for courses.
  • Manages medical emergencies according to health service plans, attending doctor appointments and transporting residents to medical appointments and emergency rooms as needed.
  • Identifies and reports maintenance issues to supervisors or through NetFacilities if available.
  • Reports to work promptly and on time, observing policies governing leave time.
  • Completes Medication Administration and Proxy training, administering meds to residents and following MAR entry policies.
  • Tracks vital signs and other health-related information for residents.
  • All other duties as assigned.
  • Must be able to lift and move up to 50lbs with or without accommodations.

  • Ability to prioritize tasks and delegate when necessary.
  • Ability to function well in high-pressure environments.
  • Proficient with Microsoft Office Suite or related software.
  • Ability to use technology to record notes.

Salary Information: The estimated annual salary for this position is $45,000-$55,000, based on location and industry standards.
