REPORTS TO: Case Management Supervisor, Regional Supervisor

CLASSIFICATION: Full Time

STATUS: Exempt

SALARY: 52,000-56,000

Position Summary: The "Home Study/Post Release Services" initiative, managed by Lutheran Immigration and Refugee Service, delivers essential support to unaccompanied immigrant minors following their release from custody. The HS/PRS Case Manager is responsible for conducting home assessments, managing cases, and providing supportive services to these children once they are placed with their sponsors in the metropolitan area.

Key Responsibilities:

  • Conduct home assessments and maintain communication with caregivers to evaluate the safety and appropriateness of the child's living situation while linking them to necessary community services.
  • Engage in ongoing case management, focusing on identifying and referring children to services that address their needs in areas such as placement, safety, education, health, mental health, legal matters, substance abuse, and personal development.
  • Provide immediate crisis intervention as required, or facilitate connections to services to ensure the child's safety and well-being during emergencies.
  • Document all interactions and assessments meticulously, adhering to organizational policies and procedures.
  • Utilize best practices in social work to assess the child's progress and the effectiveness of services rendered.
  • Maintain consistent and timely communication with supervisors regarding any concerns related to the child's safety, stability, or overall well-being.
  • Participate in regular supervision sessions.
  • Perform additional duties as assigned.

Education:

  • Bachelor's Degree in Social Work, Psychology, Sociology, or a related field with a focus on child and adolescent development is required.

Skills & Abilities:

  • FLUENCY IN SPANISH IS REQUIRED
  • A minimum of one year of experience in child welfare programs is necessary.
  • Familiarity with community resources and the ability to connect families to these services is essential.
  • Strong clinical and crisis intervention skills are crucial.
  • Excellent verbal and written communication abilities are required.
  • Strong critical assessment and analytical skills are necessary.
  • Ability to effectively manage time, prioritize tasks, and work independently.
  • Willingness to conduct home visits, which may constitute up to 70% of the role.
  • Experience working with refugee or immigrant children is preferred.
  • Cross-cultural or international experience is advantageous.
  • Knowledge of ACS/OCFS regulations related to disciplinary actions is a plus.
  • Proficiency in Microsoft Office applications is required.

Special Position Requirements:

  • A valid driver's license is mandatory.
  • Must successfully pass an FBI background check and a CA/N check for all states of residence in the past five years.
  • This position may involve a hybrid of remote and in-office work.
