Job Title: Case Manager

ICL is seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive case management services to residents, including assessment, planning, and coordination of services to meet their individual needs.

Key Responsibilities:
  • Conduct initial assessments and develop individualized service plans
  • Coordinate and provide referrals to community resources and services
  • Monitor and evaluate resident progress and adjust service plans as needed
  • Collaborate with residents, families, and other service providers to ensure a comprehensive and coordinated approach to care
  • Document resident information and service plans in a timely and accurate manner
Requirements:
  • Bachelor's degree in a human services field or related field
  • 1+ years of experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of community resources and services
