Job Title: Home Care Case Manager

LifePath Inc. is seeking a compassionate and skilled Home Care Case Manager to join our team. As a Home Care Case Manager, you will be responsible for evaluating the needs of clients, developing personalized service plans, and coordinating with healthcare providers to ensure the best possible outcomes.

Key Responsibilities:
  • Conduct comprehensive needs assessments to identify client needs and develop effective service plans.
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
  • Authorize and coordinate home care services, including home health care, adult day care, and other supportive services.
  • Monitor client progress, make adjustments to service plans as needed, and ensure timely communication with stakeholders.
  • Maintain accurate and up-to-date client records, adhering to regulatory requirements and LifePath Inc.'s policies.
Requirements:
  • Bachelor's degree in a related field, such as social work, nursing, or healthcare administration.
  • Previous experience in healthcare or social services, preferably in a case management or care coordination role.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team, with a strong focus on customer service and client satisfaction.
  • Valid driver's license and reliable vehicle for travel within the service area.
