Job Overview

ServiceNet is seeking an experienced Case Manager to join our team in our Shelter and Housing Division. Our team provides essential support to individuals and families in need, helping them achieve stability and independence.

Key Responsibilities
  • Conduct thorough intakes and assessments for program participants, identifying their unique needs and goals.
  • Develop and implement personalized service plans, ensuring participants receive the necessary support and resources to succeed.
  • Facilitate regular meetings with participants to review progress, set objectives, and make referrals as needed.
  • Collaborate with shelter staff to maintain a positive and supportive living environment.
  • Maintain accurate and detailed records of participant interactions and progress.
Requirements
  • Bilingual candidates are encouraged to apply.
  • High school diploma or equivalent, plus additional training equal to two years of college.
  • One year of experience working with individuals or families in need, or those affected by mental health issues and/or addiction.
  • Knowledge of local social service agencies and resources.
  • Valid driver's license and acceptable motor vehicle record.
