Job Summary

We are seeking a highly skilled and compassionate Patient Navigator to join our team at the University of Miami Health System. As a Patient Navigator, you will play a vital role in facilitating patients' access to healthcare services, identifying barriers to care, and coordinating treatment plans.

Key Responsibilities
  • Conduct patient evaluations to gather information necessary for effective treatment planning
  • Ensure patients have access to insurance and social service programs for which they are eligible
  • Provide patient reminders of upcoming appointments and serve as a liaison between patients and providers
  • Identify and resolve real or perceived barriers to care
  • Educate and counsel patients on health-related issues and life situations
  • Maintain current knowledge of community health resources and programs available to patients
  • Adhere to University and unit-level policies and procedures, safeguarding University assets
Requirements
  • High School diploma or equivalent required
  • Minimum 3 years of relevant work experience required; experience in a medical office or hospital preferred
  • Ability to process and handle confidential information with discretion
  • Effective communication and interpersonal skills
  • Ability to handle difficult and stressful situations with professional composure
