Position Overview:

We are seeking a full-time Care Coordinator to become a vital member of our IHPA South Chicago Team.

Compensation:

The salary for this role is competitive and commensurate with relevant experience and qualifications, with an average salary of $47,000.

Key Responsibilities:
  • Conduct outreach and engagement services to connect clients with suitable integrated services based on their needs.
  • Utilize the IHPA population health management platform (HealthEC) to track care tasks and identify members requiring targeted interventions due to high emergency room visits or hospital admissions.
  • Provide education and training to Metropolitan Family Services staff and community partners regarding care coordination, workflows, and processes.
  • Monitor daily Admission Discharge Transfer (ADT) feeds for IHPA members, ensuring follow-up with clients at emergency rooms or hospitals and facilitating post-discharge appointments.
  • Document all follow-up care and vital information in both the MFS electronic health record and IHPA platform after each interaction.
  • Participate in member education initiatives, including the development of materials and conducting presentations.
  • Collaborate with multidisciplinary teams, including Managed Care Organization (MCO) Care Coordinators, to deliver coordinated care that addresses the complex needs of members.
  • Maintain accurate client records and complete necessary agency and IHPA program reports.
  • Engage in required training and meetings.
  • Perform additional duties as assigned.
Qualifications:
  • Bachelor's degree in Social Work or a related Human Services field is required.
  • Experience working with diverse populations with significant behavioral health needs is preferred.
  • Familiarity with care coordination and Managed Care Organizations (MCO) is advantageous.
  • Valid driver's license, access to a vehicle, and proof of auto insurance are required.
  • Bilingual in Spanish is preferred.
  • Strong oral and written communication skills, with proficiency in Microsoft Office Suite and Google tools.
  • Dependable, punctual, and capable of working flexible hours, including evenings and weekends.
  • Ability to work effectively with diverse individuals and groups.
  • Self-motivated, detail-oriented, and capable of producing high-quality work independently.
  • Demonstrated commitment to social justice, community empowerment, and advocacy.
Physical Requirements:

This role requires the ability to sit and stand for extended periods, communicate effectively, and occasionally lift or move up to 50 pounds. Close visual acuity is necessary for tasks such as data analysis and computer work.
