Job Summary

We are seeking a skilled Patient Care Associate to join our team at Northwell Health. As a Patient Care Associate, you will play a vital role in providing direct care to patients, working collaboratively with our healthcare team to ensure exceptional patient outcomes.

Key Responsibilities
  • Admission and Discharge Processes
    • Assist with the admission and discharge processes, ensuring a smooth transition for patients and families.
  • Interpersonal Relationships
    • Establish and maintain good interpersonal relationships with families, visitors, and hospital staff.
    • Communicate vital information to all levels of the healthcare team, as needed.
    • Convey necessary information to patients and visitors.
    • Maintain effective communication with co-workers for proper team functioning.
  • Direct Patient Care
    • Perform direct patient care as assigned for designated groups of patients.
    • Obtain and document patient vital signs using appropriate equipment.
    • Apprise the Registered Nurse and healthcare team of any observed significant changes in the patient's condition.
    • Communicate written and verbal information.
    • Respond to patient requests and/or call lights.
    • Maintain a safe, comfortable, and therapeutic environment for patients and families in accordance with hospital standards.
    • Function in an emergency situation as a team member with guidance from supervisory personnel.
    • Assist with restrained patients according to established policy.
  • Decubitus Care and Activities of Daily Living
    • Perform grade I and II decubitus care as directed.
    • Perform activities of daily living appropriate to the patient's condition.
    • Use proper body mechanics, obtain help, and/or use appropriate equipment when lifting, moving, or transferring patients.
  • Delegated Patient Care Procedures
    • Perform delegated patient care procedures as directed.
  • Specimen Collection and Diagnostic Tests
    • Obtain and label specimens.
    • Perform certain diagnostic tests consistent with established procedures, as directed.
  • Phlebotomy and Telemetry
    • Perform phlebotomy procedures.
    • Perform telemetry and electrocardiograms as directed.
  • Clinical Knowledge and Judgment
    • Use clinical knowledge in the care and management of patients based on diagnoses and the unit assigned.
  • Additional Responsibilities
    • Perform related duties as required.
Qualifications
  • High School graduate or equivalency diploma, required.
  • Nursing Assistant Certificate with proficiency in performing EKG and Phlebotomy, preferred.
  • Minimum two (2) years of direct patient care experience, required.
