Position Overview: Wise Program Care Coordinator

Therapeutic Health Services (THS) has been a leader in delivering evidence-based behavioral health solutions since 1972. Our mission is to support children, adolescents, and families dealing with substance use and mental health issues. As a Wise Program Care Coordinator, you will play a crucial role in enhancing the lives of participants and their families by evaluating their strengths and needs, orchestrating WISe Teams, and overseeing the advancement of care plans.

Key Responsibilities:
  • Evaluate the strengths and requirements of participants and their family members.
  • Facilitate WISe Teams and hold regular collaborative meetings.
  • Establish connections among various systems that participants engage with.

Qualifications:

  • Bachelor's degree in social services or a related discipline.
  • A minimum of 2 years of experience in direct service with youth and families.
  • Completion of CANS and WISe training is required.
  • Possession of reliable transportation.
  • WA state Peer Counselor certification to be obtained within 6 months of employment.
  • Full WISe training to be completed within 6 months of employment.
