Renal Transplant Coordinator

Mass General Brigham is seeking a highly skilled and experienced Renal Transplant Coordinator to join our team. As a key member of our transplant program, you will be responsible for coordinating the care of patients undergoing renal transplantation, from pre-transplant evaluation to post-transplant follow-up.
Key Responsibilities:- Coordinate patient care from pre-transplant evaluation to post-transplant follow-up, ensuring seamless transitions between care settings.
- Collaborate with multidisciplinary teams, including transplant surgeons, nephrologists, and other healthcare professionals to ensure comprehensive patient care.
- Develop and implement individualized care plans, taking into account patient-specific needs and goals.
- Provide education and support to patients and their families, promoting self-management and adherence to treatment plans.
- Manage and coordinate patient flow, ensuring timely and efficient care delivery.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Graduate of an approved school of nursing with current registration in Massachusetts.
- Minimum two years of experience in renal medicine-focused patient care, with a strong background in transplant coordination.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Ability to maintain confidentiality and handle sensitive patient information with discretion.
