Job Summary

The PACE Community Liaison plays a vital role in managing the intake and enrollment process for new program participants, while providing and coordinating information for all inquiries about the program and/or the community at large.

Key Responsibilities
  • Receive and generate referrals from agency staff and community sources, establishing and maintaining working relationships with prospects and referral sources.
  • Develop and distribute program materials and literature to inform the community and prospects, serving as a liaison between prospects, caregivers, and community referral sources.
  • Coordinate meetings and special gatherings in support of the PACE Program with community agencies, following up with all potential participants and referrals regarding program enrollment.
  • Explain the PACE program to interested persons, including candidates for enrollment, caregivers, or others in the community, ensuring that prospective participants understand that joining the program is strictly voluntary and that they can disenroll at any time.
  • Verify current program eligibility as stated in state and federal regulations, maintaining accurate documentation for all intake/enrollment of participants.
  • Provide any additional relevant information to the Interdisciplinary Team, filing documentation to coordinate initial assessment of potential participants, and maintaining open communication with Program Management.
  • Adhere to all State, Federal, and Company regulations and policies pertaining to marketing activities for the PACE program, providing excellent customer service at all times to internal and external customers.
  • Recommend and implement new/revised policies, procedures, and systems for the program that improve efficiency, effectiveness, and minimize loss ratio.
Requirements
  • Bachelor's degree or above, preferably in Social Work, with a Master's degree preferred.
  • 3-5 years of experience in Social Work, with at least one year of experience working with frail elderly and their families, preferably in a healthcare or community agency setting.
  • Ability to travel in the community, with a valid driver's license, automobile insurance, and reliable transportation.
