Job Summary:

We are seeking an experienced Care Coordinator to join our team at Liberty Healthcare and Rehabilitation Services. As a Care Coordinator, you will play a vital role in ensuring the social needs of our patients are met.

Key Responsibilities:

  • Coordinate patient discharges in accordance with established policies and state and federal regulations.
  • Monitor and ensure completion of relevant sections of the Minimum Data Set (MDS).
  • Develop and implement a meaningful social program tailored to the needs of each patient.
  • Refer patients and families to external agencies or resources when necessary.
  • Collaborate with Unit Directors, RN Supervisors, and residents/families to facilitate admissions, discharges, and room changes.
  • Assist with new resident admissions, completion of related paperwork, and acclimation to the facility.
  • Support families or residents with discharge placement and provide necessary information.

Requirements:

  • Four-year degree and one year of experience in the healthcare field or an equivalent combination of education and experience.
  • Knowledge of local, state, and federal regulations related to patient social needs.
  • Admission and discharge planning experience is preferred.
