Transplant Nurse Coordinator

Job Summary:
The Transplant Nurse Coordinator (TNC) plays a vital role in the transplant program at UNC Health Care. This per diem RN position is responsible for coordinating the total plan of care for transplant patients from referral to post-transplant follow-up.
Key Responsibilities:
- Collaborate with the transplant program administrator to develop and implement short- and long-term plans for the program.
- Participate in continuous quality improvement efforts to maximize patient and graft survival outcomes.
- Coordinate patient care, including pre-operative testing, transplant candidacy evaluation, and post-transplant follow-up.
- Develop and maintain educational materials and present educational programs to patients, staff, and the community.
- Collect and maintain clinical data for research and reporting purposes.
Requirements:
- Bachelor's degree in Nursing (BSN) from a state-accredited school of professional nursing.
- Licensed to practice as a Registered Nurse in the state of North Carolina.
- Three years of professional nursing experience, including two years in the area of specialization.
Work Environment:
This is a per diem position with a standard 8-hour workday. The work schedule is day shift, and the location is UNC Medical Center in Chapel Hill, NC.
