Clinical Care Coordinator

We are seeking a skilled Clinical Care Coordinator to join our team in the Progressive Care Unit. This role requires an individual with strong communication and interpersonal skills to provide high-quality patient care.
Key Responsibilities:- Deliver exceptional patient care, working collaboratively with the interdisciplinary team to achieve optimal outcomes.
- Conduct thorough assessments and develop individualized care plans, ensuring patients receive tailored treatment.
- Administer medications and treatments as prescribed by physicians, maintaining accuracy and attention to detail.
- Communicate effectively with patients, families, and colleagues, fostering open dialogue and respect.
