Position Overview:
We are currently seeking candidates for the role of Care Coordinator I or II.

Compensation:
Hourly rate: $21.88 (Medical Assistant) - $22.63 (Bachelor's or LPN)

Role Summary:
This full-time position is available at our Medical Clinic. We are in search of either a Medical Assistant or a Licensed Practical Nurse (LPN) to join our clinical care team. The Care Coordinator plays a vital role in enhancing the quality of patient-centered care.

The responsibilities include assessing care gaps for patients with chronic conditions and/or mental health needs, and collaborating with the clinical care team to develop care plans during daily meetings.

Key duties involve assisting patients with medication management, navigating insurance access, and identifying additional preventive health needs. The Care Coordinator will also support patients in setting ongoing self-management goals using Motivational Interviewing techniques. Proficiency in computer skills is essential for tracking patient adherence to care plans in electronic records.

This role also requires the Care Coordinator to facilitate team meetings, necessitating strong organizational and communication skills.


Qualifications:
- Certified Medical Assistant (Care Coordinator I)
- LPN with experience in outpatient care (Care Coordinator II)
- BA/BS/BSW in a health-related field with at least one year of experience in community health.
- Bilingual in English and Spanish is mandatory.
