Transform Lives as a Case Manager II

Telecare Corporation is seeking a dedicated and compassionate Case Manager II to join our team. As a primary support and case manager, you will work directly with individuals with mental illness and other co-occurring disorders, empowering them to live independently in their chosen community.

Key Responsibilities:
  • Provide direct support and case management services to individuals with mental health needs
  • Assess and develop individualized service plans to promote independence and community integration
  • Collaborate with healthcare providers, family members, and community resources to ensure comprehensive support
  • Facilitate access to mental health services, housing, employment, and education opportunities
  • Monitor and evaluate service effectiveness, making adjustments as needed

Requirements:
  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups
