Job Title: Homecare Case Manager

We are seeking a highly skilled and compassionate Registered Nurse to fill the role of Homecare Case Manager. As a key member of our team, you will be responsible for providing exceptional homecare services to individuals in need.

Responsibilities:
  • Conduct comprehensive assessments of clients' medical needs and develop individualized care plans.
  • Coordinate all aspects of care delivery and ensure continuity of services.
  • Provide hands-on training and supervision to home care nurses and aides.
  • Collaborate with interdisciplinary team members to optimize client outcomes and promote holistic care.
Qualifications:
  • Current licensure as a Registered Nurse (RN) in the state of Washington.
  • Minimum of one year of clinical experience, with a preference for candidates with prior experience in homecare or case management.
  • Strong assessment and critical thinking skills, with the ability to develop and implement individualized care plans.
  • Excellent communication and interpersonal abilities, with a compassionate and client-centered approach to care.
