Position Overview
We are enhancing our clinical operations team to accommodate our increasing client and patient demographic. We are seeking full-time Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs)/Licensed Practical Nurses (LPNs) to engage in remote physiological monitoring, development and management of patient care strategies, and to offer support and guidance to our patient community. This role is entirely remote, allowing you to utilize our proprietary software platform to maximize patient engagement. If you are passionate about delivering patient care and are looking to minimize time spent on documentation, this opportunity is tailored for you.
Key Responsibilities
Nurses will operate from a remote setting. The primary duties include:
- Evaluating patient vitals and care strategies within our proprietary technology platform.
- Creating and overseeing patient care strategies to address clinical, social, and behavioral requirements.
- Responding to alerts in patient vitals to enhance health outcomes.
- Delivering education and guidance to promote health through video, telephonic, and asynchronous interactions.
- Acting as a liaison between our physician clients and their patients to facilitate access to care.
- Coordinating care with healthcare providers.
- Assisting patients in cultivating healthy habits to improve their health over time.
Qualifications
Applicants must possess a valid nursing license, with multi-state licensure preferred. Proficiency in additional languages is advantageous. Previous experience in case management is beneficial, as is familiarity with motivational interviewing techniques. Exceptional customer service skills, comfort with new technology, and the ability to work remotely are essential.
