We are seeking a highly skilled Clinical Nurse Supervisor to lead our nursing team on the 11pm to 7am shift at Southtowns Care Center.

Job Summary:

The Clinical Nurse Supervisor will be responsible for overseeing the day-to-day clinical activities of our healthcare personnel, including LPNs and CNAs. This role requires strong leadership and communication skills, as well as the ability to interpret patient information and make critical decisions.

Key Responsibilities:

  • Conduct admissions assessments and reassessments on all patients
  • Provide leadership and supervision to the nursing staff, housekeeping, and security
  • Work closely with medical support staff to ensure resident needs are met
  • Greet admitted patients and prepare them for physical examination and treatment
  • Maintain medical supplies and equipment
  • Provide education as indicated after assessing the learning needs of patients and staff members
  • Assess patients' physical and psycho-social conditions on an ongoing basis throughout their care continuum
  • Utilize manual and automated information systems to improve efficiency and productivity
  • Accurately record patient information in electronic medical records systems and patient medical records
  • Demonstrate a current working knowledge of equipment, instrumentation, and medications
  • Act as a liaison in coordinating patient care activities with nurses, physicians, and allied health professionals
  • Participate in preparation and facilitation of regulatory agency site visits, including DOH
  • Maintain adequate infection control standards in accordance with agency protocol and DOH standards
  • Reviews and reports all medical, accident/incident-related to patient care, and provides input into necessary actions to resolve problems
  • Participate in weekly and as-needed case conferencing sessions with interdisciplinary team members to discuss resident issues/concerns
  • Evaluate and document changes in residents' conditions
  • Ensure that personnel adhere to established policies and procedures
  • Document changes in resident's conditions
  • Participate in care planning meetings
  • Mentor and supervise LPNs and CNAs

Requirements:

  • Current NYS Registered Nurse license
  • 3 years RN experience preferred
  • Current BLS Certificate preferred
