Job Summary:

The Nurse Supervisor is responsible for delivering high-quality patient care in a collaborative environment. This role involves working closely with primary care providers, nursing teams, and patients to ensure seamless transitions of care. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for serving diverse patient populations.

Key Responsibilities:
  • Collaborate with healthcare teams to improve patient outcomes and provide quality care.
  • Ensure all clinical care meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, and patient satisfaction.
  • Perform physical exams and patient assessments to identify and address health problems and unmet patient care needs.
  • Triage patient concerns and immediate needs by phone or in person.
  • Provide nursing intervention and health education for individuals, families, and/or other groups regarding disease prevention, childbirth, support programs, and other health-related concerns.
  • Collect samples from patients and perform routine laboratory tests.
  • Prepare patients for examination, testing, or treatments and explain procedures.

Requirements:
  • Graduation from an accredited nursing program with a current NY state licensure as an RN.
  • Hybrid Option available after probationary period.
  • Direct experience in a primary care (preferred but not required).
  • Reliable transportation between sites required.
  • Current CPR and Basic Life Support certification (BLS).
  • Maintain credentials, attend seminars, and fulfill all licensure requirements including continuing education.

Working Conditions:
  • Must be able to sit or stand for long periods of time.
  • Exert up to 50lbs. pushing, pulling, or lifting.
  • Walking, climbing stairs required.
  • Able to talk/hear and communicate with others with or without reasonable accommodation.

Travel Requirements:
  • Must be able to travel to sites while maintaining standards of safety and compliance with motor vehicle traffic laws.
  • This position may require travel for necessary training and conferences.
