Professional Case Management Senior Case Manager RN

This full-time position offers the opportunity to work as a senior leader in our home health care team. The ideal candidate will have strong clinical knowledge and leadership skills.

About the Role

As a Senior Case Manager RN, you'll oversee the delivery of care to our clients, manage a team of RN Case Managers, and participate in performance improvement initiatives. You'll also ensure compliance with agency policies and state regulations.

Responsibilities

This role involves:

  • Directly overseeing the delivery of home care to clients
  • Managing a team of RN Case Managers
  • Providing direct client care and support
  • Participating in performance improvement initiatives
  • Ensuring compliance with agency policies and state regulations
Requirements

To be successful in this role, you should have:

  • A graduate degree from a state-approved school of professional registered nursing
  • Minimum two years of nursing experience, including one year in home care or a closely related field
  • One year of supervisory and/or case management experience preferred
Salary and Benefits

We offer a competitive salary of $100 hourly plus shift differentials, where applicable. Benefits include flexible scheduling and compensation for drive time.
