Job Description:

The successful candidate will be responsible for providing comprehensive patient care coordination, including:

  • Conducting thorough and accurate nursing assessments to identify patient needs and develop individualized care plans;
  • Collaborating with healthcare professionals to ensure seamless care transitions and optimal patient outcomes;
  • Preparing equipment and aids for practitioners to facilitate efficient and effective care delivery;
  • Observing, recording, and reporting patient conditions in a timely and accurate manner;
  • Developing and implementing policies and protocols that promote quality patient care and safety;
  • Maintaining organized, clean, efficient, and confidential work areas to ensure a positive patient experience.
Requirements and Qualifications:

To be considered for this exciting opportunity, candidates must possess:

  • A valid license to practice as a Registered Nurse or Licensed Practical Nurse in the State of Washington;
  • Strong communication and interpersonal skills to effectively collaborate with patients, families, and healthcare teams;
  • Ability to think critically and solve problems in a fast-paced clinical environment;
  • Familiarity with electronic medical records and other relevant software systems.
