Job Summary

The RN Care Manager is a vital member of our healthcare team, working collaboratively with physicians and other healthcare professionals to improve the health outcomes of patients with complex needs. This role requires a strong clinical background, excellent communication skills, and the ability to navigate multiple systems to ensure seamless care coordination.

Key Responsibilities
  • Conduct comprehensive assessments of patients' health needs, identifying potential barriers to care and developing personalized care plans.
  • Collaborate with the primary care physician and patient care team to establish and implement care plans.
  • Provide education and support to patients and families, empowering them to manage their health effectively.
  • Identify patients at risk and develop proactive interventions to prevent hospitalizations and emergency department visits.
  • Coordinate care for complex cases, working with multiple specialists and healthcare providers to ensure optimal outcomes.
  • Maintain accurate and up-to-date documentation of patient assessments and care plans, ensuring seamless communication among healthcare team members.
  • Analyze patient care trends and collaborate with the care team to improve overall quality and efficiency of care.
Requirements
  • Associate's degree in nursing (ADN/ASN) or higher.
  • Current RN license in the state of practice.
  • Minimum of two years of clinical experience in an outpatient setting or two years of experience coaching individuals with complex and/or chronic conditions.
Preferred Qualifications
  • Bachelor's degree (BS/BA/BSN).
  • Prior experience in care coordination in a community setting or primary care practice.
  • Program development, operations management, and/or supervisory experience.
Physical Requirements

This role requires the ability to see and read information, labels, and assess patient needs. The employee must be able to verbally communicate and hear and understand spoken information, alarms, and needs quickly and accurately. Manual dexterity is required to manipulate complex equipment, and the ability to lift and utilize full range of movement to transfer patients is necessary. The employee must also be able to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment.
