Stillwater Hospice is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing high-quality patient care and coordinating services to meet the needs of individuals and families in their homes and communities.
Key Responsibilities:- Conduct comprehensive assessments of patients and families to determine hospice needs.
- Develop and implement individualized care plans that establish goals and incorporate palliative nursing actions.
- Provide education and support to patients and families on self-care techniques, medication management, and other health-related topics.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Supervise and delegate responsibilities to ancillary personnel as needed.
- Graduate of an accredited school of nursing with 1-2 years of recent acute care experience.
- Current licensure in the state and a valid driver's license.
- Excellent observation, verbal, and written communication skills, problem-solving skills, and basic math skills.
- Prolonged or considerable walking or standing, ability to lift, position, and/or transfer patients, and ability to lift supplies and equipment.
