RN Care Manager

The Nurse Care Manager plays a vital role in improving the health of patients with chronic conditions or complex needs. This position requires collaboration with physicians and other healthcare team members to ensure optimal care.
Key Responsibilities:
- Perform comprehensive assessments of patients' health needs, including health status, behaviors, and available support systems.
- Establish care plans in collaboration with the primary care physician and the patient care team.
- Provide health education and identify patients at risk for proactive intervention.
- Pull and manipulate data to identify risk patients and present information to the care team.
- Refer patients to various resources, including nutrition, social work, rehabilitation, and behavioral specialists.
- Coordinate care for complex cases or those patients seeing multiple specialists.
- Maintain up-to-date and accurate documentation of patient assessment and plan.
- Analyze patient care trends and actively seek out and collaborate with the care team to improve overall quality and efficiency of care.
Requirements:
- Associates Degree in Nursing (ADN/ASN)
- Current RN license in the state of practice
- Minimum of two (2) years of clinical experience in an outpatient setting or a minimum of two (2) years of experience coaching individuals with complex and/or chronic conditions
Preferred Qualifications:
- Bachelors Degree (BS/BA/BSN)
- Prior experience in care coordination in a community setting or primary care practice
- Program development, operations management, and/or supervisory experience
Physical Requirements:
The Nurse Care Manager must be able to see and read information, labels, and assess patient needs. They must also be able to verbally communicate and hear and understand spoken information, alarms, and needs quickly and accurately. Additionally, they must be able to lift and utilize full range of movement to transfer patients and push/pull gurneys and portable equipment.
