Job Summary

We are seeking a highly skilled Registered Nurse Care Coordinator to join our team at the Billings Pediatric Specialty Clinic. As a key member of our healthcare team, you will work collaboratively with physicians and other healthcare professionals to provide exceptional patient care and improve health outcomes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' health needs, including health status, behaviors, and available support systems.
  • Develop and implement individualized care plans in collaboration with the primary care physician and patient care team.
  • Provide health education and coaching to patients and families to empower them to manage their health care needs.
  • Identify patients at risk for proactive intervention and develop strategies to address their needs.
  • Collaborate with payers and outside agencies to promote patient-centered delivery concepts.
Requirements
  • Associate's Degree in Nursing (ADN/ASN)
  • Current RN license in the state of practice
  • Minimum of two years of clinical experience in an outpatient setting or two years of experience coaching individuals with complex and/or chronic conditions
