Job Title: Social Worker

The Social Worker is a vital member of our team at Rutland Regional Medical Center, responsible for providing social work services to customers affected by illness, disability, or lifestyle adjustment difficulties. This role requires a deep understanding of utilization criteria, case management principles, and relevant community resources, insurance, regulatory, and legal system knowledge.

Key Responsibilities
  • Conduct psychosocial assessments and utilize case management principles to develop and implement effective care plans.
  • Facilitate and co-facilitate psychosocial and skill-building groups to support patients and their families.
  • Provide individual and family supportive advocacy to ensure patients receive necessary resources and services.
  • Collaborate with interdisciplinary teams, including healthcare providers, legal representation, regulatory agencies, and risk and compliance teams, to ensure comprehensive care.
  • Develop and maintain knowledge of community resources, insurance, and regulatory requirements to provide patients with optimal support.
Requirements
  • Bachelor's degree in Social Work (BSW) or a related field such as Psychology or Human Services.
  • Two years of experience in social work, medical crisis intervention, and community resources, or a related field.
  • BLS Certification through a reputable organization.
  • Completion of a patient/visitor de-escalation and restraint program certification.
Compensation

The salary range for this position is $47,500 - $75,800.
