Clinical Pharmacist

As a Clinical Pharmacist at Kaiser Permanente, you will play a vital role in ensuring the safe and effective use of medications for our patients. Under general supervision, you will dispense compounds, procure, store, and distribute pharmacy products, while also providing direction to pharmacy interns and non-licensed personnel. Your primary focus will be on developing and sustaining productive relationships with patients and colleagues, while actively seeking information to understand patient circumstances, problems, expectations, and needs.
Key Responsibilities:- Quality and Safety: Know and comply with all Kaiser Permanente quality, safety, and emergency policies and procedures. Demonstrate quality and effectiveness in work habits and clinical practice in every interaction with patients, colleagues, providers, and leadership. Ensure patient safety in the preparation and provisioning of care related to medications, procedures, infection prevention, and fall prevention.
- Dispensing and Compounding: Dispense, compound, procure, store, package, and distribute pharmacy products according to legal requirements, established policies, and procedures. Document responsibility by signing or initialing prescriptions, batch sheets, or other forms.
- Drug Therapy Evaluation: Evaluate and resolve potential drug therapy problems identified through any available sources, including pharmacy information systems.
- Leadership and Training: Provide direction to pharmacy interns, technicians, and other personnel by making suggestions, answering questions, resolving professional and clerical problems, orienting new employees, training employees in new procedures, and checking their work for errors.
- Patient Counseling: Interview patients to obtain information regarding drug use, allergies, and sensitivities. Document the information in the patient's medical record or pharmacy information system. Advise patients on significant precautions, proper drug therapy, and administration.
- Collaboration with Medical Personnel: Confer with medical personnel concerning patient care and treatment, including clinical diagnosis, drug combinations, and dosage forms.
- Record Keeping: Maintain records of drugs and controlled substances as required by federal and state law. Maintain records pertinent to the purchase and control of narcotics and alcohol.
- Problem-Solving: Accept responsibility for resolving operational problems, such as resolving professional problems related to prescription interpretation, drug furnishing irregularities, and service complaints.
- Equipment Maintenance: Check equipment used to prepare and dispense medications for proper functioning and report deficiencies to appropriate personnel.
- Work Environment: Maintain a safe and efficient work environment by keeping the work area and equipment organized and clean.
- Customer Service: Establish and maintain courteous and cooperative relations with the public, patients, and other personnel.
- Professional Development: Keep abreast of all current drug information as it relates to medicines, their uses, dosages, and inherent side effects.
- Education: Graduation from an accredited school of pharmacy with completed pharmacy internship.
- License: Pharmacist License (Colorado).
- Additional Requirements: Effective verbal and written communication skills, ability to authorize a D.E.A. drug screening, and demonstrated customer service skills.
