Job Overview The Clinical Pharmacist General Medicine ensures safe, appropriate, and economical use of drugs through the application of specialized skills and knowledge in pediatric patient care. This position will provide clinical pharmacy services to support our growing medical and surgical areas. This pharmacist will join two (2) other General Medicine Clinical Pharmacists to broaden the services currently being provided and lead the development of the pharmacist role in service expansion. Additional Information Department: Pharmacy Hours per week: 40, eligible for benefits Shift: Day, with rotating weekends and holidays. Qualifications Education: Doctor of Pharmacy or Bachelor of Science in Pharmacy degree. Experience: Completion of one year pharmacy residency. Equivalency: One year of pharmacy work experience accepted in lieu of residency. Licensure: State of Colorado Department of Regulatory Agencies (DORA) Pharmacist license. Previous pediatric and residency training is preferred. Responsibilities POPULATION SPECIFIC CARE Neonate - 30 days to 1yr Toddlers - >1yr to 3yrs Pre-Schoolers - >3yrs to 5yrs School age - >5yrs to 13yrs Adolescent - >13yrs to 18yrs Adult - >18yrs to 65yrs ESSENTIAL FUNCTIONS An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform. Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal, compounding, therapeutic uses of medications and pharmaceutical supplies in a timely manner; using a systematic approach to effectively searching, retrieving, and evaluating the literature and appropriately communicating and applying the information to the patient care situation; and assisting with the development, maintenance, and implementation of hospital and department policy and procedures rela ed to medication use, storage, restrictions, and administration Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Reports adverse drug events and medication variances using the hospital variance reporting program (QSRS). Sustains clinical pharmacy services throughout the organization by providing pharmaceutical care for pediatric patients of all age ranges through participation in multi-disciplinary team rounds and/or care, emergency
