Job Overview The Clinical Pharmacist Specialist ensures safe, appropriate, and economical use of drugs through the application of specialized skills and knowledge in pediatric patient care. This position will be supporting anticoagulation coverage and transitions-in-care for the Heart Institute and Hemophilia and Thrombosis Center. The Department of Pharmacy provides comprehensive clinical services to patients at Children's Hospital Colorado. Our clinical pediatric pharmacists are available 24 hours a day and help ensure the safe and effective use of medication in children - as well as promote health, wellness and disease prevention. Additional Information Department: Pharmacy Hours per week: 40, eligible for benefits Shift: Monday - Friday, 0700-1530. Every 3rd weekend rotating and selected holidays. Qualifications Education: Doctor of Pharmacy or Bachelor of Science in Pharmacy degree. Experience: Two (2) years of residency experience or one (1) year of residency plus three (3) years of work experience. Equivalency: Three (3) years of pharmacy work experience in area of specialty accepted in lieu of 2 nd year of residency. Certifications: Board of Pharmacy Specialties (BPS) Certification required within 3 years of start date in position. Licensure: State of Colorado Department of Regulatory Agencies (DORA) Pharmacy license. Responsibilities POPULATION SPECIFIC CARE a.) Neonate - 30 days to 1yr c.) Toddlers - >1yr to 3yrs d.) Pre-Schoolers - >3yrs to 5yrs e.) School age - >5yrs to 13yrs f.) Adolescent - >13yrs to 18yrs g.) Adult - >18yrs to 65yrs ESSENTIAL FUNCTIONS Designs and leads pharmaceutical outcomes research. Identifies opportunities for and participates in drug utilization reviews and research. Demonstrates sustained record of contributing to the pharmacy practice knowledge through publications in professional journals or books, posters/presentations at professional meetings, and review of book chapters and manuscripts submitted for publication. Participates in the education of pharmacy staff, nursing staff, medical staff, pharmacy residents and students, other healthcare allied professionals and patients and families. Participates on a hospital committee(s). Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal. Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Sustains clinical pharmacy services throughout the organization by providing pharmaceutical care for pediatric patients of all age ranges. Participate in general pharmacy operations throughout the organization. Other Information SCOPE AND LEVEL Advanced full performance professional level work that requires additional knowledge, education, training or experience. COMPLEXITY: Duties performed involve concepts, theories, and concrete factors to be evaluated and weighed requiring a high degree of analytical ability, independent judgment, and decision-making. DECISION MAKING: Exercises judgment and discretion, and is responsible for determining the time, place and sequence of the work performed. COMMUNICATION: Contacts with team members, clients or the public where some degree of discretion and judgment are required. SUPERVISION RECEIVED: Performs work with minimal supervisory oversight.
