Key Responsibilities

  • Assess the patient's functional capabilities, overall health status, and physical therapy requirements, collaborating with physicians to formulate a comprehensive therapy plan.
  • Deliver services within the legal scope of practice as defined by state regulations, adhering to the established plan of care while coordinating with the healthcare team.
  • Monitor and communicate the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
  • Administer treatments aimed at alleviating pain, restoring function, and maximizing patient capabilities using evidence-based practices.
  • Educate and inform all involved parties, including healthcare team members, patients, and their families, about the treatment plan, functional limitations, and progress towards objectives.
  • Conduct patient visits throughout the designated service area, as well as nearby locations as necessary.
  • Complete progress notes for each visit in accordance with company policy, ensuring timely documentation.
  • Perform comprehensive patient evaluations and develop tailored physical therapy plans in compliance with state guidelines, reporting findings and anticipated treatment frequency to the supervising nurse within 24 hours.
  • Participate in team meetings and committees as requested by the organization.
  • Supervise home health aides in accordance with company policy and state regulations, if permitted.
  • Oversee the activities of Physical Therapist Assistants as per company policy and state law.
  • Engage in weekend rotations to provide services as needed across the agency.
  • Prepare written discharge summaries and condition reports at least every 60 days.
  • Provide in-service training as requested by leadership.
  • Adhere to established Therapy Standards of Practice in delivering services to all patients.
  • Take responsibility for pursuing continuing education to maintain and enhance clinical expertise and ensure compliance with state licensure requirements.
  • Fulfill any additional duties as assigned.

Qualifications

Licensure Requirements

  • Valid Physical Therapy license in the state of Louisiana.
  • Current CPR certification.
  • Possession of a valid driver's license, vehicle insurance, and access to reliable transportation.
