About the Role

We are seeking a skilled Physical Therapist to join our team at Ochsner Home Health LaPlace. As a Physical Therapist, you will play a vital role in helping patients achieve their maximum potential and improve their quality of life.

Key Responsibilities
  • Evaluate patients' functional status and physical therapy needs, and develop a personalized plan of care in collaboration with the healthcare team.
  • Provide high-quality physical therapy services to patients in their homes, adhering to the plan of care and state laws governing the practice of physical therapy.
  • Monitor patients' progress, report any changes in their condition to the nurse supervisor and/or physician, and adjust the plan of care as needed.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in staff conferences and committees to share knowledge and best practices.
  • Meet productivity and quality standards, and adhere to LHC Therapy Standards of Practice.
Requirements
  • Current Physical Therapy licensure in the state of Louisiana.
  • Current CPR certification.
  • A valid driver's license and vehicle insurance, and access to a dependable vehicle.
