Your responsibilities will include providing culturally sensitive, patient-focused care, crafting individualized care plans, and acting as a liaison between clients and service providers.

  • Develop and implement care plans tailored to the needs of individuals and families.

Qualifications:
  • High School Diploma or equivalent preferred

  • Prior experience in volunteer work, community service, or healthcare is advantageous
  • Certification as a Promotor(a) or Community Health Worker, or willingness to obtain within 6 months
  • Bilingual skills (English/Spanish) are preferred

Work Environment:

This position involves working in office or clinical environments, where proficiency in using office equipment, tools, and electronic devices is essential. Compliance with safety and security protocols, including the use of required equipment, is mandatory.
