Key Responsibilities
  • Assist patients in accessing health resources and services, including mental health services and community outreach.
  • Support disease self-management and coordinate culturally appropriate care and services for patient populations.
  • Represent the program in the community and work with individuals, families, groups, and community partners to provide outreach, public education, and referrals as needed.
  • Develop and maintain relationships with community partners to enhance program effectiveness.
  • Participate in program development, implementation, and evaluation to ensure alignment with community needs and goals.
Requirements
  • High school diploma or equivalent.
  • Valid Maine driver's license and a clean driving record.
  • Personal vehicle available for work use.
  • Lived experience in the community being served.
  • Bilingual in English and any languages specific to the communities being served (if relevant).
  • Resident of the community being served with good knowledge of local resources.
  • Experience in outreach, customer service, social service, or related field (preferred).
  • Fluency in specified language (verbal and written).
  • Ability to carry out written and oral instructions.
  • Comfortable with home visits and community or targeted outreach to patients who share a common background with the candidate.
Preferred Qualifications
  • Experience in translation (if relevant to the community being served).
  • Comfortable with Microsoft Office suite.
