We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Aveanna Healthcare. As a LPN, you will have the opportunity to provide high-quality patient care in a home healthcare setting, working closely with our patients and their families to achieve optimal health outcomes.

Job Summary

The LPN will be responsible for delivering and coordinating quality patient care in compliance with physician orders, observing and assessing patient condition and care needs, and documenting all activities, assessments, and nursing actions in a timely manner. The ideal candidate will have a strong commitment to patient-centered care, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities
  • Deliver and coordinate quality patient care in compliance with physician orders
  • Observe and assess patient condition and care needs
  • Document all activities, assessments, and nursing actions in a timely manner
  • Participate in the development and implementation of the nursing care plan
  • Provide education to patients and families regarding disease process, self-care techniques, and prevention strategies
Requirements
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
Physical Requirements
  • Must be able to speak, write, read, and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must be able to sufficiently reposition patients and move equipment without assistance
