Job Title: Private Duty Nurse LPN

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Private Duty Nurse. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our team to ensure the best possible outcomes.

Job Summary

The Private Duty Nurse LPN will be responsible for delivering skilled nursing care to patients in their homes, under the supervision of a Registered Nurse. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities
  • Provide and document skilled nursing care to patients in their homes, in accordance with physician orders and developed care plans.
  • Conduct regular assessments of patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
  • Develop and implement individualized care plans, in collaboration with the patient and their family.
  • Administer medications and treatments as prescribed, and document patient responses and outcomes.
  • Provide education and support to patients and their families, promoting self-care and independence.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
