Nurse Case Manager

We are seeking a skilled and compassionate Nurse Case Manager to join our team at Mass General Brigham. As a key member of our healthcare team, you will play a vital role in ensuring the seamless transition of patients from hospital to home, while also promoting high-quality, patient-centered care.
Key Responsibilities- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare providers, patients, and families to ensure effective communication and coordination of care
- Conduct thorough assessments of patients' medical, social, and emotional needs
- Identify and address potential barriers to care, such as social determinants of health and healthcare access
- Provide education and support to patients and families to promote self-management and empowerment
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Current licensure as a Registered Nurse in the Commonwealth of Massachusetts
- Minimum of three years' clinical experience in acute care or case management
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
- Ability to work independently and as part of a team, with a focus on collaboration and coordination
- Basic computer skills, including proficiency in electronic health records and other relevant software
