Job Title: Registered Nurse Home Care Specialist

Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our team at Royal Care Certified. As a Registered Nurse Home Care Specialist, you will provide high-quality nursing care to patients in their homes, promoting their well-being and facilitating their speedy recovery.

Responsibilities:

  • Provide visiting home health services to an assigned caseload, including admissions, re-certification, and discharges.
  • Develop the Physician Plan of Care in coordination with all health care professionals assigned to the case.
  • Provide documentation using a tablet within the required 24-48 time frame.
  • Be knowledgeable in the use of the current OASIS D1 assessment for all patient admissions, re-certification, transfers, and discharges.
  • Ensure that all home care services are provided in a high-quality manner and in accordance with Department of Health and Government regulations.

Requirements:

  • Proven experience as a registered nurse.
  • Valid nursing license.
  • Pediatric experience.
  • Must have a vehicle and can drive.
  • Must have experience completing comprehensive OASIS assessments.
  • Experience in acute care in a hospital setting or home care experience.
