Work Schedule: Full Time (40 hours/week), typically 8 AM - 5 PM, Monday to Friday (may include occasional evenings and weekends)
Compensation: $21.65 - $25.70 per hour, based on experience

Essential Duties:
- Conduct comprehensive assessments with participants to determine demographics, income, housing status, and individual needs.
- Collaborate with participants to create and implement personalized action plans addressing various life domains such as Housing, Income, Education, Wellness, and Social Capital.
- Engage with a diverse range of participants, ensuring culturally responsive support.
- Utilize program funds effectively based on individual goal plans and assess participation duration based on needs.
- Employ assertive engagement and trauma-informed care techniques to motivate individuals toward their goals.
- Maintain regular contact with households to monitor progress and provide ongoing support.
- Research community resources and advocate for families' needs, including housing stability and legal assistance.
- Facilitate financial discussions to help families manage their resources effectively.
- Assist families in their housing search, addressing barriers and supporting application processes.
- Ensure compliance with documentation standards required by funding sources.
- Stay informed about regional housing laws and tenant rights.
- Support families receiving Housing Vouchers by liaising with relevant agencies.
Secondary Duties:
- Build and maintain relationships with community partners, including landlords.
- Conduct follow-ups with households as required by funding sources.
- Participate in supervision, staff meetings, and training sessions.
- Engage in community outreach and interagency collaborations.
- Perform additional duties as assigned, demonstrating flexibility and teamwork.
The ideal candidate will possess:
- A Bachelor's Degree in social services or a related field, with at least six months of case management experience, or equivalent education and experience totaling at least one year.
- Experience in developing and implementing individualized service plans.
- Background in crisis intervention and community resource networking.
- Experience in culturally responsive service delivery using assertive engagement techniques.
- Fluency in English, both written and spoken; bilingual skills are preferred.
- Proficiency in Microsoft Office applications.
The role requires the ability to perform various physical tasks, including lifting up to 25 pounds and maintaining a moderate noise level in the work environment. Reasonable accommodations will be made for individuals with disabilities.
Transportation:A valid driver's license, clean driving record, and reliable transportation are required for this position.
