Optum Home and Community Care: A Leader in Integrated Care

At Optum, we're revolutionizing the way healthcare is delivered. Our Senior Community Care (SCC) product is a pioneering effort to provide holistic care to patients in their homes, nursing homes, and assisted living facilities. As a Nurse Practitioner or Physician Assistant, you'll be part of a team that's changing the face of healthcare.

Primary Responsibilities:
  • Primary Care Delivery: Provide high-quality, cost-effective care to assigned members, managing both medical and behavioral conditions effectively in collaboration with physicians or specialty providers.
  • Care Coordination: Understand payer and plan benefits, CCM associate policies, and articulate them effectively to providers, members, and key decision-makers. Assess medical necessity and coordinate care as members transition through different levels of care and care settings.
  • Program Enhancement: Regularly communicate with internal and external parties, promote the CCM program in assigned facilities, and develop new and improved methods to address concerns and issues.
Requirements:
  • Certified Nurse Practitioner through a national board, with preferred certification as ANP, FNP, or GNP.
  • Active and unrestricted license in the state of residence.
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Access to reliable transportation for travel to client and/or patient sites.
  • Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions.
Preferred Qualifications:
  • 1+ years of hands-on post-grad experience within Long Term Care.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
  • Proficient computer skills, including documentation of medical information with written and electronic medical records.
