Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team at Optum. As a key member of our interdisciplinary team, you will provide high-quality patient care to seniors in their homes, nursing homes, and assisted living facilities.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, mental, and social needs
  • Develop and implement individualized care plans to address patients' complex health needs
  • Manage patients through acute medical situations, chronic condition exacerbations, and short-term rehabilitation episodes
  • Ensure accurate and complete documentation of patient information, including ICD-10 conditions
  • Communicate and collaborate with the interdisciplinary care team to coordinate patient care
  • Provide patients and caregivers with education and counseling on health management and disease prevention
  • Facilitate agreement and implementation of patient care plans with facility staff, families, and primary care clinicians
  • Coordinate patient care transitions across different levels of care and care settings
Requirements
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners) or accredited Physician Assistant degree program (for Physician Assistants)
  • Active and unrestricted Nurse Practitioner or Physician Assistant license in the state of practice
  • Board certification through the American Academy of Nurse Practitioners or American Nurses Credentialing Center (for Nurse Practitioners) or National Commission on Certification of Physician Assistants (for Physician Assistants)
  • Ability to gain a collaborative practice agreement, if applicable in the state of practice
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions
  • Availability to work 24 hours per month, with expectations that 16 of the 24 hours/month could be during off-hours
  • Access to reliable transportation
Preferred Qualifications
  • New graduates are encouraged to apply
  • 1+ years of clinical experience in senior living, geriatric medicine, or home care
  • Experience in meeting the healthcare needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records
