As a Nurse Practitioner or Physician Assistant, you'll be part of a team that's changing the face of healthcare.
Primary Responsibilities:- Primary Care Delivery: Provide high-quality, cost-effective care to assigned members, managing both medical and behavioral conditions effectively.
- Care Coordination: Facilitate seamless transitions between care settings, ensuring that members receive the right care at the right time.
- Program Enhancement: Promote the CCM program in assigned facilities, partnering with key stakeholders to drive membership growth and improve outcomes.
- Active and unrestricted license in the state of practice
- Current DEA licensure/prescriptive authority or ability to obtain post-hire
- Access to reliable transportation for travel to client and/or patient sites
- Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions
- Availability to work 24 hours per month, with expectations of 16 hours during off-hours
- 1+ years of hands-on post-grad experience in Long Term Care
- Understanding of Geriatrics and Chronic Illness
- Proficient computer skills, including documentation of medical information
