The Clinical Coordinator is responsible for overseeing the care plans for patients in hospital and nursing home settings, ensuring a seamless transition from admission to discharge. Key Responsibilities:
  • Facilitate the admission process by reviewing patient records and conducting orientations.
  • Track daily admissions and discharges to support statistical analysis and cost management.
  • Conduct intake interviews to assess client needs for medical and psychological evaluations.
  • Develop and implement treatment plans, coordinating necessary services and resources.
  • Monitor patient progress, advocating for necessary services and intervening during crises.
  • Maintain comprehensive patient records, documenting case notes and progress.
  • Collaborate with management to ensure appropriate follow-up care for discharged patients.
  • Provide weekly updates on nursing home patients and identify potential high-risk cases.
  • Coordinate patient transfers as directed by medical professionals.
  • Assist in arranging direct admissions to hospitals and placements in skilled nursing facilities.
  • Monitor length of stay for patients and facilitate discharge planning.
  • Conduct interdisciplinary meetings to communicate patient progress and treatment outcomes.
  • Enhance staff skills through educational resources and training opportunities.
  • Analyze treatment processes to improve outcomes and financial performance.
Qualifications:
  • Demonstrated experience in case management, preferably as an RN or in a related healthcare role.
  • Strong understanding of case management principles and reimbursement processes.
  • Excellent communication and organizational skills.
  • Ability to manage multiple tasks and solve problems effectively.
  • Familiarity with medical terminology, including CPT codes and ICD 10.
  • Bilingual candidates are preferred.
Minimum Requirements:
Active RN license in Florida with a minimum of three years of experience in a clinical coordination role.
