Job Summary

We are seeking a highly skilled and compassionate Intensive In-Home Qualified Professional to join our team at Southeastern Integrated Care LLC. As a key member of our staff, you will provide critical support to individuals with mental health needs, working closely with our team to develop and implement personalized care plans.

Key Responsibilities
  • Person-Centered Plan Development: Collaborate with clients, families, and healthcare professionals to create and update comprehensive care plans that address individual needs and goals.
  • Case Management: Coordinate services, link clients with community resources, and facilitate communication between healthcare providers to ensure seamless care delivery.
  • Advocacy and Support: Provide emotional support, advocacy, and education to clients and their families, promoting empowerment and independence.
  • Documentation and Reporting: Accurately document client interactions, progress, and outcomes, ensuring timely and thorough reporting to stakeholders.
  • Team Collaboration: Work closely with our multidisciplinary team to share knowledge, best practices, and resources, enhancing the overall quality of care.
Requirements
  • Education and Experience: Bachelor's degree in a human services field or related field, with a minimum of 2 years of experience working with mental health populations.
  • Skills and Abilities: Excellent communication, documentation, and interpersonal skills; ability to work independently and as part of a team; flexibility in responding to changing client needs.
  • Certifications and Licenses: Valid NC driver's license and personal vehicle insurance coverage; current license or certification in a relevant field (if applicable).
Work Environment

This role involves working primarily in clients' homes and communities, with occasional travel to our office. You will be exposed to various environments, including domestic settings, and may encounter challenging situations. However, you will also have the opportunity to make a meaningful difference in the lives of our clients and their families.

Physical Demands

This position requires regular standing, walking, and use of hands to interact with clients and documents. You will also need to lift and move up to 20 pounds occasionally and up to 50 pounds rarely. Specific vision abilities required include close vision, distance vision, and the ability to adjust focus.
