We are seeking a highly skilled and compassionate Qualified Professional/Case Worker to join our team at Southeastern Integrated Care LLC. Key Responsibilities- Develop and implement Person-Centered Plans (PCPs) in collaboration with clients, families, and healthcare professionals.
- Provide direct and indirect interventions to support clients in achieving their treatment goals.
- Coordinate services and referrals to ensure seamless care transitions.
- Advocate for clients' rights and interests, promoting autonomy and self-advocacy.
- Collaborate with multidisciplinary teams to ensure comprehensive care planning and coordination.
- Document all interactions, interventions, and progress in a timely and accurate manner.
- Participate in ongoing training and education to enhance skills and knowledge in mental health, developmental disabilities, and substance abuse.
- Bachelor's degree in a human services field or related field.
- 2+ years of experience in mental health, developmental disabilities, or substance abuse services.
- Valid NC driver's license and reliable transportation.
- Ability to work independently and as part of a team.
- Excellent communication, documentation, and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
This position requires frequent travel to clients' homes and community settings. You must be able to work in a fast-paced environment and adapt to changing circumstances.
