Key Responsibilities
  • Develop and implement Person-Centered Plans (PCPs) in collaboration with clients, families, and healthcare professionals.
  • Provide direct and indirect interventions to support clients in achieving their treatment goals.
  • Coordinate services and referrals to ensure seamless care transitions.
  • Advocate for clients' rights and interests, promoting autonomy and self-advocacy.
  • Collaborate with multidisciplinary teams to ensure comprehensive care planning and coordination.
  • Document all interactions, interventions, and progress in a timely and accurate manner.
  • Participate in ongoing training and education to enhance skills and knowledge in mental health, developmental disabilities, and substance abuse.
Requirements
  • Bachelor's degree in a human services field or related field.
  • 2+ years of experience in mental health, developmental disabilities, or substance abuse services.
  • Valid NC driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
  • Excellent communication, documentation, and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
Work Environment

This position requires frequent travel to clients' homes and community settings. You must be able to work in a fast-paced environment and adapt to changing circumstances.
