We are seeking a highly skilled and compassionate Intensive In-Home Qualified Professional/Case Manager to join our team at Southeastern Integrated Care LLC. As a key member of our Intensive In-Home Team, you will play a vital role in providing high-quality, person-centered care to individuals and families in need.
Key Responsibilities- Provide Intensive In-Home Services, an enhanced mental health service, to children and adolescents with serious and chronic symptoms of emotional, behavioral, and/or substance use disorders.
- Assist individuals and families in developing critical daily living and coping skills, and provide support in implementing interventions identified in the Person-Centered Plan.
- Work collaboratively with the Intensive In-Home Team, including the Team Leader, to ensure effective delivery of services and achieve treatment goals.
- Participate in clinical supervision and adhere to individualized supervision plans and contracts.
- Document daily progress notes and maintain accurate records of services provided.
To be successful in this role, you will need:
- A Bachelor's degree in a Human Services field, or a related field with 2 years of post-bachelor's degree experience in MH/DD/SAS population, or
- A Bachelor's degree in a field other than Human Services with 4 years of post-bachelor's degree experience in MH/DD/SAS population, or
- A Master's degree in a Human Services field with 1 year of post-graduate degree experience in MH/DD/SAS population, or
- A valid driver's license and personal vehicle insurance coverage.
