We are currently in search of a qualified:
HOSPICE LICENSED PRACTICAL NURSE (LPN)
Full Time ***$5,000 Sign-on Bonus***
Position Overview:
As a Hospice Licensed Practical Nurse, you will:
1. Deliver direct patient care, utilizing a range of skills and coordinating necessary services.
2. Complete required documentation in accordance with policies, procedures, and regulations.
3. Implement the care plan tailored to the patient's diagnosis and condition as established by the Registered Nurse.
4. Collaborate with the Registered Nurse, Patient Care Coordinator, and other agency staff to enhance patient care, while engaging with the patient, family, community organizations, and physicians.
5. Accept and accurately transcribe physician's orders as instructed.
6. Relay all clinical, progress, and summary notes to the Hospice Registered Nurse for care coordination.
7. Participate actively in Interdisciplinary Team (IDT) meetings.
Qualifications:
1. Successful completion of nursing education from an accredited nursing school.
2. Current LPN licensure in the relevant state and a minimum of one year of clinical nursing experience, preferably in an acute care medical/surgical environment.
3. Ability to independently manage workload and deliver effective healthcare services to hospice patients.
4. Capacity to ensure appropriate patient care under the supervision of the Hospice Registered Nurse, with thorough, accurate, and timely documentation.
5. Strong communication skills, both verbal and written.
6. CPR certification is required.
