Job Description

As a Home Health Physical Therapist Assistant with Interim HealthCare of New Hampshire, you will have the opportunity to make a meaningful difference in the lives of patients who require personalized therapy in the comfort of their own homes.

Key Responsibilities

* Provide high-quality, patient-centered physical therapy services to individuals who have lost mobility, strength, and functionality due to illness or injury
* Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement individualized plans of care
* Conduct thorough assessments, observe patient movements, and document progress to inform care decisions
* Assist patients with exercises and hands-on therapy to manage pain and increase mobility
* Educate patients and their families on therapy goals, exercises, and self-care strategies

Requirements

* Active PTA license in the state of Maine
* Minimum of 1 year of PTA experience, preferably in home healthcare
* CPR certification
* Up-to-date on all state and federal home health regulations
* Strong clinical judgment, interpersonal skills, dexterity, and compassion
