Home Health Physical Therapist Assistant

Interim HealthCare of New Hampshire is seeking a dedicated and compassionate PTA to join our team.

As a Home Health Physical Therapist Assistant, you'll have the opportunity to work with a diverse range of patients, each with their unique needs and goals. You'll work closely with our team of healthcare professionals to develop and implement personalized care plans, using your skills and expertise to help patients achieve optimal outcomes.

We're looking for a PTA who is passionate about delivering high-quality care, has excellent communication skills, and is able to work independently with minimal supervision. Responsibilities:
  • Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
  • Work as part of a home health team to develop and implement personalized care plans
  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients on their plan of care, established goals, and self-care
  • Coach family members on the plan of care, therapy goals, and safely assisting with exercises
Requirements:
  • Active PTA license in the State of New Hampshire
  • Minimum of 1 year of PTA experience, ideally in home healthcare
  • CPR certification
  • Up-to-date on all State of New Hampshire required vaccinations
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion
